Majora Leviathora 84^^^ drops it.
Mob is found in Chelsith (at -340, 30, -60).

Praetor of the Phylactery 80^^^ drops it as well.
Mob is found in Charasis: Vault of Eternal Sleep (at Altar of the Dragonslayer).

We don't know, yet. We know the item exists, but our source does not contain what quest it comes from, or what mobs drop it. We just know it is out there.
