Each quest must be completed 4 times, at which point you will not be able to get the quest from the item anymore. Once the final quest is completed, examining the item will give you this quest.

Pretty simple, return to Neophyte Sairal ( 1649.17, -267.84, 3200.46 ) in Paineel to turn in the quest and receive your choice of reward.
