Confirmed, page 11 dropped from a lamia's body at location -84.11, -1.56, -1182.39

I found page 5 as a ground spawn at 13.33, -1.52, 122.85. It's on the beach by Lamia's and there are several other pages that spawn in this area.

You can find the wall page 10 in the Enchanted Lands on the Island of the Pine Trees. (north east where the high lv nightbloods are.) Click on a tree 'lantern'. (clickable.)

Page 4 is on a cart (turned on its side) at -768, ,-956. Right beside the Nightblood Tower.
