This quest is initiated by examining one of the coins that is rewarded to you for completing the Cleansing the Filth quest in Stormhold.

You must click the Memorial of Vengeance, which is one of the pillars near the entrance in Fallen Gate. It will ask you to collect the blood from 8 zombies in Fallen Gate. Afterwards, click on the urn at the base of the pillar to drop the offering in and complete the quest.
